In order for your child to be re-enrolled in Horizon Christian Schools for the 2012-2013 school year, you will need to complete 3 steps. Please note you will need a current version of Adobe Acrobat Reader in order to view, fill-out, save, and print these forms. If you do not have the most current version of this free software already installed on your computer, you may encounter difficulty with opening and viewing the documents. Please go to the following website and download it before trying to access Horizon’s forms.
If it better meets your family’s needs, please be aware that all documents posted on this website are also available in paper form at each school’s front office. You are welcome to request a Re-Enrollment Packet and other materials from office staff and complete the paperwork by hand if you are not able or do not desire to print the forms yourself.
Step 1: Download and complete the 2012-2013 Re-Enrollment Form for EACH child that will be re-enrolling with Horizon Christian Schools.
Step 2: Download and complete a Volunteer & Driver Screening Form for EACH parent/guardian.
• If you have previously volunteered at Horizon, please download and complete the yellow Renewal Form: Background and Driver Screening Form RENEWAL
• If you have not previously volunteered at Horizon, please download and complete the blue Application Form. A copy of you driver’s license must be submitted with this form before it is considered complete. For your convenience, the school can make a photocopy of your license if you visit either campus’ front office: Background and Driver Screening Form
Step 3: Submit a Registration Fee in the form of cash or check for the required amount, based upon the 2012-2013 Registration and Open House Timeline.
When downloading forms please note they are in a fillable PDF format, which allows you to type directly into the document fields. You are then able to save any typed information and return to the form on your computer at any time.
If you are filling out forms for multiple children, save and print the form, then open the saved file, click "save as" and rename it as a new document. You can then type over the information that needs to be changed for each child and save and print again.
You also have the option of printing the documents and filling them out by hand.
In either instance, once all documents have been completed in their entirety, please attach them together and submit them with the registration fee to your campus’ front office.
PLEASE NOTE: Applications will not be accepted until all required pieces are present.
